IRadimed came into this Q3 2026 report with a premium reputation and a premium P/E multiple near 41x, priced as a high quality medical equipment growth story. The stock then dropped about 11% on the first trading day after results. That sharp pullback lands against a quarter that delivered record revenue of US$24.2m and net income of US$6.5m, both supported by a very high gross margin of 76%.
The expectation gap is clear. Trading has punished the shares in the short term while the latest numbers highlight a business still printing strong profitability.

IRadimed’s new 3870 pump is already driving record revenue. Ask Charlie what the 3870 pump means for IRadimed’s growth.
The bullish story hangs on 3870 driven replacement demand and richer mix translating into better quality earnings for IRadimed.
"A large multi year replacement opportunity in the roughly 6,400 older pump channels, combined with management’s target of more than 2,000 3870 channels sold annually, points to sustained system demand."
Q3 showed real progress toward that ambition. Pump revenue reached US$8.7m, helped by more than 400 3870 systems produced and over 300 shipped, with ASP about 23% to 25% above the 3860. Gross margin at 76% improved sequentially, which supports the idea that higher value quad systems can scale without eroding profitability.
The bear side focuses on whether the 3870 ramp could undercut margins and earnings even as units climb.
"The initial 3870 production ramp has already led to a lower gross margin compared with a year earlier because of higher manufacturing costs and inefficiencies, and if the effort to restore past manufacturing efficiency takes longer than management expects, IRadimed could face sustained pressure on gross margin and net income even if top line growth remains intact."
Q3 offered partial relief but not a full escape from that concern. Gross margin recovered to 76% and rose about 2 points versus Q2, yet management again highlighted quality controlled ramping and trimmed the top end of full year revenue guidance to avoid stressing production, which shows the manufacturing constraint is still shaping outcomes.

IRadimed just showed how a strong income statement can still meet a sharp price reaction. The real opportunity is spotting stocks that could be undervalued before the market closes that gap.
