Establishment Labs Holdings just watched its stock slide 8.8% to about US$62 in the first trading session after reporting Q3 2026. The initial reaction is harsh. Yet the headline from the quarter is not collapse. It is a cosmetic implants specialist putting up roughly US$72 million in sales and, for the first time, generating positive free cash flow of US$5.1 million.
Short-term traders see a falling share price. Long-term holders are staring at a business that is still loss making on a GAAP basis but now generating cash and lifting full year revenue guidance.

U.S. sales more than doubled and drove a guidance raise. Ask Charlie what made Establishment Labs’ U.S. growth explode.
Bulls frame Establishment Labs as a higher value procedure engine, not just a volume story.
"Preservé procedures that allow surgeons to charge 30 to 50% more than traditional augmentation, and more than a 2x procedural premium in the U.S., create room for Establishment Labs Holdings to tilt the mix toward higher ticket minimally invasive cases that support structurally higher gross margins and potentially stronger net margins."
Q3 gives real evidence. Minimally invasive revenue reached about US$9 million and roughly 15% of year to date sales, with Preservé about 17% of U.S. turnover. Adjusted gross margin of 70.8% and the first quarter of positive free cash flow indicate the mix is starting to work, even with distributor heavy OUS revenue diluting margin optics.
Skeptics worry that U.S. uptake and margin ambition baked into the story could prove too optimistic.
"The main thing that has to go right is continued rapid U.S. and direct-market adoption of Motiva and the minimally invasive platform, so that higher-margin procedures and operating leverage keep translating into sustained gross margins above 70% and positive adjusted EBITDA."
Current numbers argue partly against that fear. U.S. sales of US$26 million grew much faster than OUS regions and now contribute about 36% of total revenue. Minimally invasive turnover more than doubled, and adjusted EBITDA reached US$6.4 million with adjusted gross margin just above 70%. The share price fall of about 9% suggests investors still question how repeatable these milestones are.
