Investors who held Merck over the past year are up 66.5%, including dividends. That gap between messy accounting results and a powerful gain is hard to ignore. The bullish narrative saw Fair Value at US$102, or 14% above the start price. It relied on revenue growth of 4.2%, a profit margin assumption of 33.7%, and a future P/E of 12.3x. Over 20 planned product launches and a much larger late-stage pipeline were expected to do the heavy lifting.

The more cautious view put Fair Value at US$74, or 17% below the start price. That narrative treated revenue growth of 2.8%, a 27.3% margin, and a future P/E of 12.1x as more realistic, while flagging risks around acquisitions and the impact of Keytruda’s eventual patent expiry.

What The Evidence Changed For The Merck Narratives

The clearest event for Merck was the Q2 2026 report. Revenue moved from US$15,806m in Q2 2025 to US$16,607m in Q2 2026, which helped the growth assumption in the bullish case. Profit told a different story. Net income excluding extra items turned from US$4,427m to a loss of US$1,335m and net margin shifted from 28.0% to a negative 8.0%. That profitability gap meant the evidence cut both ways.

The lesson is simple. When a thesis leans on higher margins, investors should look beyond sales or the product pipeline. It can be useful to track net income and the reported net margin each period to see whether the profit side of the story is actually reflected in the numbers.

What Merck's Run Already Prices In

Merck now trades at US$144 after a 66.5% gain over the past year, and the selected Narrative sees Fair Value below that level. The gap reflects concern that the recent surge prices in a smoother path through patent expiries and late stage development risk than the evidence guarantees.

A buyer here is effectively assuming KEYTRUDA pressure is largely contained and the next wave of oncology, cardiometabolic and flu products fills the earnings gap. The open question is how the specific safety, regulatory and uptake risks in this Narrative line up against that confidence.
