The “Action Plan” states that it is necessary to rely on urban medical groups and county medical communities to gradually establish a vertical integrated geriatric care linkage model between medical institutions within the medical consortium, promote the extension of institutional geriatric care services to the community and home, and improve the nursing service system covering acute diagnosis and treatment of diseases in the elderly, chronic rehabilitation, stable care, and end-of-life palliative care. The “Action Plan” requires expanding the coverage of “Internet+ Nursing Services” and making full use of information technology to promote door-to-door nursing services for elderly patients with reduced mobility, such as disability, disability, etc. At the same time, the “Action Plan” suggests that by 2027, nurses specializing in geriatric care in the district will account for more than 95% of the training.
